Safe Access Zones Survey

The Northern Ireland Department of Health are producing their annual report setting out whether each safe access zone in Northern Ireland has been effective in protecting the safety and dignity of protected persons. 

Can you please fill out the following feedback form regarding your experiences with Safe Access Zones - whether as a service user at a hospital or simply being in the vicinity.

It is really important that voices from across the north are heard in this survey.
